If you have been scheduled for a sleep consult, please fill out the Sleep Clinic Questionnaire and return to us prior to your appointment. Please bring copies of all previous sleep-related medical records to your appointment. We will need any prior records in order to provide any new testing or care options.

For your convenience, we can pre-authorize studies and clinic visits for most insurance plans. Our schedulers will call your patient to schedule his/her sleep study or clinic appointment and will notify you of the scheduled date. If you would like pre-completed referral forms, call us at 303.270.2708.

You should receive a faxed report of the results of your patient's sleep study or clinic evaluation within 7-10 business days.

If a patient needs to be seen for insomnia by Jack Edinger, PhD, who specializes in cognitive behavioral therapy, a referral with a diagnosis of insomnia from a patient’s physician must be sent to the National Jewish Health Sleep Center, regardless of insurance.
